Complete the survey online in five easy steps!
• Go to NYCSchoolSurvey.org.
• Click on the link to the “Take the 2025 NYC School Survey” button.
• Enter your access code (lowercase “f” followed by your student’s identification (OSIS) number which can be found on their report card, student ID card or NYCSA account.).
• Complete the survey.
• Click ‘Submit’.
